Devereux Advanced Behavioral Health Arizona’s Community Based Services program provides in-home remedial intervention for at-risk youth and families identifying youth and family strengths and needs to enhance the Child and Family Team (CFT) planning process. We work with families and youths in their homes, teaching a variety of skills ranging from self-care, household management, living, and social. We are looking for talented individuals who have experience working in Behavioral Health, with families and at-risk youth, to be a positive role model and impact on our families!
Reporting to the Program Manager, the Family Support Coordinator (Crisis Support Coordinator) is responsible for the coordination of care, treatment planning, service delivery, and case management of our placement preservation and crisis stabilization services. This position formulates treatment plans, assessments, clinical impression, and discharge planning. The Crisis Support Coordinator also provides support to individuals, families, and placements to help stabilize behaviors and avoid disruption. You will work closely with the Child/Family Team to identify the supports needed to ensure stabilization and maintain stability successfully moving forward.
Pay Range: $19 - $20.90 / hr
Flexible Full Time Schedule: Tuesday - Saturday or Sunday - Thursday; ability to work afternoons/nights
Qualifications
Minimum requirement of high school diploma or equivalency. Bachelors degree preferred
High school diploma or high school equivalency diploma at least four (4) years of full time behavioral health work experience; OR:
Has a high school diploma or equivalency diploma and a combination of education in a field related to behavioral health and full time behavioral health work experience totaling at least two (2) years; OR:
Has an Associate's degree and at least two (2) years of full time behavioral health work experience; OR:
Has a Bachelor's degree and at least one (1) year of full time behavioral health work experience
OTHER:
Must be at least 21 years of age or older (per licensing standards)
Must be available to attend two (2) weeks of new hire orientation (paid, Mon-Fri 8:30am-5pm) plus 2-3 weeks of on the job training
Drug test within 24hrs of offer w/ negative results

Valid Arizona Driver's License & pass DMV 36-month history check
Current AZ Fingerprint Clearance Card with no restrictions (or eligibility to obtain with no restrictions)
